Despite the urgings of national health organizations for women to have annual or biannual mammograms after the age of 40, uncertainty and controversy about the procedure persists. Based on cumulative evidence, screening mammography has become standard health care in many countries. However, the value of the procedure has been challenged by two Danish researchers who reviewed the major clinical trials of screening mammography declared that five of the seven trials were flawed and that none demonstrated that it saved lives.

The report, originally published in the Lancet in January 2000, was written by Peter Gotzsche and Ole Olsen from the Nordic Cochrane Center in Copenhagen. It found that most of the seven studies reviewed were invalid, in part because they failed to assign women to screened and non-screened groups. The two studies that did randomize women correctly, the researchers said, showed no value to mammography. Cancer experts from around the world overwhelmingly denounced this report. Even the researchers' own institution distanced itself from the report, stating that the findings had not been submitted to the Nordic Cochrane Center's usual rigorous review.

After the storm of criticism, the Danish authors have reevaluated their original data and declared that it "confirmed and strengthened" their original conclusions, stating that "screening mammography is unjustified because there is no reliable evidence that it reduces mortality."

Many women are now wondering if they should continue to have annual mammograms. After all, for many years women were told to do self examinations of their breasts, but recently that advice was discontinued after a large study found it completely ineffective at reducing the death rate from breast cancer.

However, other experts have challenged the iconoclastic report and contend that mammography is lifesaving. The American Cancer Society found no reason to alter its recommendation for annual mammographic screening for women over 40 years of age. The National Cancer Institute found that the screening test contributed to a pronounced drop in the death rate from breast cancer. The study, published in The New England Journal of Medicine, concludes that 28 to 65 percent of the sharp decrease in breast cancer deaths from 1990 to 2000 was due to mammograms. The remainder was due to powerful new drugs to treat breast cancer.

In the era since mammography has become widely used, the size of tumors at the time of detection has declined along with the death rate from breast cancer. During the early 1980's, when only 13 percent of women in the United States received mammograms, the average tumor size at detection was about three centimeters. By the late 1990's, 60 percent of women were having regular mammograms, and the average tumor size had shrunk to two centimeters, according to data from the Cancer Society and the National Center for Health Statistics.

This seemingly small difference in tumor size is extremely significant in terms of prognosis. On average, the larger a tumor is, the longer (or faster) it has been growing, with a subsequent correlation between tumor size and metastasis. More aggressive, debilitating treatment must be undertaken against large tumors, and even with this, survival chances of women with large tumors are worse than those of women with smaller tumors.

Of course, there are always exceptions, and some small tumors are aggressive and spread early, while some larger tumors are slow-growing and remain localized. Currently, it is impossible to distinguish between them by mammography. Even after biopsy and microscopic exam, it is difficult to predict how a tumor will grow.

There have been claims made that the recent reduction in breast cancer mortality, nearly 2 percent a year since 1990, is due to improved drugs and treatment. However, there is evidence that early detection has made a major contribution. A 29-year follow-up of breast cancer deaths in two Swedish counties published in 2001 revealed a 63 percent decline in the death rate from breast cancer in women who were offered mammography, and no decline in the death rate in unscreened women, even though the same improved treatments were available to all.

Currently, all major U.S. medical organizations recommend screening mammography for women 40 and older, claiming that the procedure reduces breast cancer mortality by 20 to 35 percent in women ages 50 to 59 and slightly less in women 40 to 49 at 14 years follow-up.

Mammography is an imperfect screening tool. It sometimes produces false-negative results: in women under 50, mammography is likely to miss 20 to 25 percent of existing cancers, and in women over 50, it misses 8 to 10 percent. Because of this, when a woman or her physician feels a suspicious breast lump that does not appear on a mammogram, it should be examined by some other means, such as biopsy.

For most of us it has meant some additional hours on the job or maybe even taking a second job if there is no overtime available with our primary employer. Others have gone down the sometimes questionable route of multi level marketing and internet marketing in an attempt to secure that all important extra income.

Don't misunderstand, many folks make a sizable full time living from MLM and internet based business working from home. But unfortunately for most of the people that venture into this online world the end result is not so positive. Instead of earning an extra income they actually lose money and for many of these people who are already struggling this is not good news. Many of them are in debt but end up investing money borrowed on their credit cards to fund programs or schemes that they know absolutely nothing about.

The biggest problem with MLM or internet network marketing schemes is how they raise peoples expectations about how much they are able to earn and how quickly they can earn it. The fact is that many people are earning money from home business opportunities but it didn't happen overnight. The good news is that once these businesses get going people out there make them a full time career and they never look back.

The trick to earning an extra income is to decide whether you want a little now or a lot later. In other words if you set your expectations at a realistic level and work consistently you have the ability to build a long term residual income for life. If you are only looking for a temporary solution to a short term financial need then read no further. The one blatant truth about MLM or internet marketing is that it does have the ability to grow your income month on month. The principal being that the work you do this month is added to the work you do next month. In other words the extra income you make is built upon month by month. In contrast to a conventional job where you work X number of hours and are paid for them. Then if you want more money you go back the next day and do it all over again.

So there you have it in it's simplest form, earning an extra income from MLM should be looked at as a long term endeavor. Set yourself realistic targets and don't expect success of any kind within the first few months. The excitement you will experience as you see those checks increase month after month will be motivation enough to spur you onwards and upwards. The best piece of advice for any budding home business entrepreneur is "be consistently persistent" and you are sure to see that extra income grow into a full time career.

It's that simple. Publishers have already drawn audiences interested in a particular topic to their site. Now you need to make sure your words make it onto those sites.

Now that you've found your market for the article you wrote about organic gardening, you need to develop that market. Rotten Bananas and Your Biz

I've finally got up the nerve to write to you about this "off-the-wall" topic. One of my favourite hobbies is baking, especially banana breads. For quite some time - about a year now - I have been too busy to find the time to do this, or so I've told myself. I always have lots of bananas on hand, and every day when I go into the kitchen I look at them and say "tomorrow I'll find some time". When tomorrow comes I say the same thing and the next day and so on... you get the picture. It's called procrastination.

What I end up doing is putting them in the fridge. Every day, they are a reminder of what I love doing but I never find the time to do and again I say "tomorrow or maybe on the weekend". At this point I am getting really mad at myself for not baking that bread.

But it gets worse. They start rotting and I move them to the freezer. My mom told me this trick. When I go into the freezer, there they are again, staring at me - rotten bananas! By now I have bananas in my freezer that are over six months old!

In deciding to write to you about this, I finally found the motivation to bake that bread, and now I can share with you the joy I felt when I did. I felt really happy when I baked it - as if I just got an A+ on an exam, or had my first date! From then on, I made a promise to never keep rotten bananas again. Finding the time to do what I've always intended to do by making my banana bread - even if it means a few extra pounds for me - made me very happy!

So why am I telling you about this? I've learned some valuable lessons from this experience that may benefit you:

1. Tell someone about your commitment to do what you want to do. That way you're accountable. Having you as my readers made me accountable.

2. Many people become too preoccupied with work and end up sacrificing their own and their family's well being. Stress builds up and they burn out. Be sure to leave at least one day a week for leisure and being with the people that are important to you. And take care of yourself - set aside time each day to do something you enjoy. When you lead a balanced life you'll enjoy good health, have a happy family life, and find peace of mind.

3. In your business, this may mean freeing up your time by delegating to others. If you don't have the expertise for the task, or you hate doing it, why not give the job to someone who has the know-how, is good at it, and actually loves doing it? You can delegate administrative or marketing tasks that bog you down to a Virtual Assistant and take back the time you need to focus on what you do best.

According to a recently published report, titled “ 360° Degree Report on Learning Management Systems,” by the eLearning Guild, a global network of more than 25,000 e-learning professionals, SumTotal® Systems, Inc. (Nasdaq: SUMT) has won Member Choice Awards for learning management system (LMS) customer satisfaction and market share, respectively. In particular, SumTotal received kudos from Guild members for its solution for small- and medium-sized corporations.

For its report, the Santa Rosa, Calif.-based eLearning Guild based market share ratings on data from over 2,300 respondents and satisfaction ratings on information from over 1,200 respondents.

Across a list of more than 135 LMS providers, eLearning Guild members gave SumTotal three awards, two Gold and one Platinum.

The first Gold award given was for achieving LMS customer satisfaction among small and medium corporations, which the eLearning Guild defined as organizations with less than 5,000 employees.

SumTotal – a global provider of talent and learning management solutions – won a second Gold award for LMS market share among small and medium corporations. According to the eLearning Guild’s report, SumTotal holds more than 20 percent of the small and medium corporate LMS market.

SumTotal earned its Platinum award for LMS Market Share among large corporations, defined as companies with at least 5,000 employees. According to the eLearning Guild’s report, one out of every three large corporations surveyed owns the SumTotal LMS.

“With 25,000 people around the world as members, we believe the eLearning Guild comprises the largest and most influential group of e-learning design, development and management professionals in the world,” said Steve Wexler, director of research and emerging technologies for the eLearning Guild. “The fact that SumTotal is the winner of the Platinum award for market share is noteworthy, indeed, as it is both a substantial and substantiated achievement.”

As part of its report, the Guild also surveyed over 930 e-Learning professionals representing 840 different organizations to definitively document the state of the LMS industry. When e-learning professionals responding to the report were asked whether or not their LMS provided “a good ROI,” of those who have had TotalLMS in place long enough to evaluate the product, 83 percent answered yes.

Well, I think it's high time we start learning how to reasearch these companies "before" we sign up. I don't think we want to feel scammed anymore.

I've been working on the Internet now for over 4 years and I have seen and signed up for "so" many programs it would make your head spin from exhaustion! I would get so frustrated after I signed up only to find that the program was just such a joke and that the program was not explained properly on their website. About a year ago, I learned how easy it was to research a particular program "before" I purchased it. Oh, I'm still checking out all the programs but now I know how to reasearch them first. You'll be surprised at how many programs I don't sign up for now because of this great little tip I'm going to tell you about.

I'll show you now how you're going to research a particular company or website and to find out what they're truly selling or offering to you. I'll give you an example below. By the way, this is not a real website.

Let's take blahblah.com for example. Go to Google.com and type this in the search box without the quotes "about blahblah.com" Google.com is one of the best search engines to find information. You can always use yahoo or one of the other search engines but I have found that Google.com is the best.

Then hit the search button. You will be amazed at how many searches come up for what you are looking for. Then scroll down until you find a forum posting. You will need to find what others are saying about this particular company and what happened to them after they signed up. Or you can try typing in this search "forum about blahblah.com"

Note: In case you don't know what a forum is. It is a website where anyone can post a message or question. Then someone else may be online reading your question and then they will respond with an answer for you. It's the best way I know of to get all kinds of tips, helpful hints or research any program or product.

You'll be surprised what you'll find on any particular website, product or service just by doing a little reasearch before you buy. You should be able to make a wise decision on signing up with a particular company after you have done your research. "For a while" is usually about 15 to 30 minutes. Often times, uninvited appointments are not a life-and-death situation, something that you can swiftly respond with, "I have just five minutes to see you, if you don't mind". That five minutes is an investment towards relationship building with your suppliers but it should not take more time than necessary. You are not obliged to see that person but it would be impolite to turn them down as it would create unhealthy image of yourself and the company.

What do you do to indicate to your co-worker that you are not to be disturbed?

Other than put a "funny" poster that conveys the message "Get Lost", you could be polite about it and say you have life-threatening deadline to meet. Or you could loudly declare that you are very stressed right now and about to explode with the

You could pack your own lunch, so that nobody can persuade you to go out and lunch

with them. On busy days, cancel your social lunch appointments if you have already

made a prior arrangement and make a sincere apology. Do not let your social

commitments eat into your working hours. This creates unnecessary stress, and you

end up going home late or worse still - bringing the work home.

How can you steal time to finish work and still get to go home early?

You come in an hour earlier. This is when no one is in the office yet, when the phone is not ringing, when your mind is still fresh and alert.
